Daily allowance of vitamin B6 ranges from 2 to 3 mg. A balanced and varied diet is enough without a need for extra supplementation with this vitamin. Vitamin B6 deficiency is extremely rare since it is widely distributed in many foods. In those cases where supplementation is required, it is recommended not to exceed 40 mg a-day. There are controversial data on the capacity of a high dose of Pyridoxine to inhibit the secretion of Prolactin and suppress the milk production. The American Academy of Pediatrics rates it as usually compatible with breastfeeding.

A parasitic fungus from the ear of some cereals, particularly rye, which since ancient times has caused epidemics of a serious disease, ergotism (Florea 2017, Belser 2013, Lee 2009). It has a complex composition, highlighting the ergoline alkaloids: lysergic acid amides such as ergometrine and ergopeptines such as ergotamine and ergotoxine (Florea 2017, Hulvová 2013) that act on adrenergic, dopaminergic and serotonergic receptors. Since the last update, we have not found published data on its excretion in breast milk. Ergot alkaloids have oxytocic, vasoconstrictive and lactation-inhibiting effects by inhibiting prolactin, among many others (Florea 2017, Hulvová 2013, Lee 2010, Lee 2009). Indications from the German Ministry of Health’s Commission E: none. Due to its serious adverse reactions and toxicity, natural alkaloids are not used in therapy, and there are recommendations for their withdrawal from sale (EMA 2014). At present, semisynthetic derivatives such as methylergometrine, dihydroergotamine, dihydroergocristine, bromocriptine and lisuride are used, among others. A polymer made out of silicon-oxygen-methyl combination with a high molecular weight, water repellent and low superficial tension. It is used in many ways (dimethicone, simethicone, -see specific items)orally to treat infant colic and flatulence; as pediculicide, in cosmetic creams and lotions and skin protectants as to prevent ulcers and scars; arthroplasties, retinal detachments and reconstruction or cosmetic surgery as injections and implants. Silicone is widely distributed in our environment with several cosmetic and medicinal uses. No evidence of toxicity on human tissues has been shown. A 1994 report on immunological side effects in infants breastfed by mothers with silicone implants, was denied categorically by means of meta-analysis and other work. The absorption by oral or dermal route is negligible. Both a high molecular weight and polymer molecular structure make it practically impossible excretion in the milk and hence a significant amount of intestinal absorption by the infant. Those circumstances make silicone implants safe for lactation even if broken or manufacturing fault (Poly Implant Prothèse, PIP). After extensive analysis of such silicone prosthesis, where lack of health risk was demonstrated, it can be concluded that many of the initial recommendations published lacked scientific validity, including that carriers of such prosthesis should not breastfeed. Silicon levels in blood and milk of women with implants (55 ng / ml) are similar to those of women without implants (51 ng / ml), 13 times lower than that found in cow's milk (709 ng / ml) and 80 times lower than in commercial infant formulas (4403 ng / ml). Thiamine or Vitamin B1 is a water soluble vitamin. In addition to Thiamine, other chemical compounds with similar activity: Acetiamine, Benfotiamine, Bisbentiamine, Bisbutiamina, Cetotiamina, Cicotiamina, Cocarboxylase, Fursultiamine, Monofosfotiamina, Octotiamine, Pyrophosphotiamine, Prosultiamine and Sulbutiamine.It is essential for the metabolism of carbohydrate nutrients. Its deficiency causes severe neuromuscular and cardiac symptoms known as Beriberi and Wernicke-Korsakoff disease. Thiamine deficiency is common among disadvantaged populations in Southeast Asia (predominantly consumers of refined rice), other malnourished people (refugees, low socioeconomic status ...) and chronic alcohol consumption. Beriberi of childhood, both infants and children, may arise from breastfeeding by Thiamine deficient mothers. Reportedly, several severe cases have occured after feeding the babies with artificial formulas that were not supplemented with vitamin B1. Daily allowance is higher during pregnancy and lactation (1.5 mg / day) which is readily obtained through a varied diet with adequate content of whole grains, legumes, nuts, eggs and lean meat.Thiamine is excreted in breast milk and gradually increases with time, being lower in colostrum (28 ng / mL) and transitional milk than in mature milk (180 ng / mL). The concentration is lower in milk from mothers of preterm (90 ng / mL). Taking vitamin supplementation is not required if diet and nutritional status are adequate. Supplementation does not increase levels in milk of well-nourished women, but of those with a low nutritional status. It has multiple industrial uses: fertilizers, explosives, metallurgy (soldered, galvanized and welded), batteries, detergents, etc. Exposure limits in these TLV industries (such as TWA) should be maintained: 10 mg / m³ and TLV (as STEL): 20 mg / m³. Ammonium chloride has no R-phrases or specific recommendations for breastfeeding. It is also used in the food industry as an emulsifying and flavoring additive (E510); to be avoided.

A fatty acid present in the body involved in metabolism which transforms glucose into energy. It is a powerful antioxidant which is synthesized in the body and also comes from external input (liver, meat, fish, green leafy vegetables, yeast). There is still no definitive scientific evidence for its usefulness in the treatment of diabetic neuropathy (Han 2012), hepatic dysfunction, certain encephalopathies, pain relief, glossodynia (Carbone 2009) and multiple sclerosis (Khalili 2014, Yadav 2010, Namaka 2008). It is generally well tolerated if the dosage and duration indicated in the manufacturer’s directions for use are not exceeded, but it can have adverse effects. Cases of autoimmune hypoglycemia syndrome (Michalopoulou 2015, Bresciani 2011, Uchigata 2007) have been attributed to its consumption, as has hepatic cholestasis (Ridruejo 2011), poisoning (Hadzik 2014, Karaarslan 2013) and seizures, especially in children (Tolunay 2015). Dermatitis has been reported due to contact with an anti-aging cream containing lipoic acid (Bergqvist 2006). Since the last update we have not found published data on its excretion in breast milk. It has been used to treat postpartum perineal pain without presenting problems during breastfeeding (Costantino 2015). Consumption in moderate doses following medical prescription would have little risk during breastfeeding. When used topically, do not apply to the nipple so as to avoid its ingestion by the infant.

Zinc (Zn) is an essential element for nutrition. It is present in many foods.Recommended daily allowance of Zn is 8 to 15 mg. (Moran Hall 2010). Millions of people worldwide are Zn-deficient.It is used as a treatment for Wilson's disease and Acrodermatitis Enteropathica. Zn is involved in the regulation process of lactation (Lee 2016).Pasteurization of the milk does not affect the concentration of Zn and other trace elements (Mohd Taufek-2016). The average concentration of Zn in breastmilk is 4 to 16 mg / L (Picciano 1976, Hannan 2005, Dórea 2012) which is independent of plasma levels and maternal daily intake (Krebs 1995, Chierici 1999, Hannan 2009).Intestinal absorption of zinc is almost doubled during pregnancy and lactation (Fung 1997).Zinc levels in the infant are dependent on Zinc levels in the breast milk (Dumrongwongsiri 2015)With a varied and balanced diet, an extra intake of minerals is not needed. Black cohosh (Cimicifuga racemosa, formerly Actaea racemosa) root was thought to have mild estrogenic activity based on its triterpene content, which is standardized based on 27-deoxyactein. In general, there is a low frequency of adverse reactions, but dizziness, nausea, headache, rash, vomiting, and rarely, hepatitis and allergic reactions have been reported.[3][10][11] Some sources recommend against its use during breastfeeding because of the lack of safety data and its potential estrogenic activity,[10] while others do not contraindicate its use.[9] Dietary supplements do not require extensive pre-marketing approval from the U.S. Food and Drug Administration. Manufacturers are responsible to ensure the safety, but do not need to the safety and effectiveness of dietary supplements before they are marketed. Dietary supplements may contain multiple ingredients, and differences are often found between labeled and actual ingredients or their amounts. A manufacturer may contract with an independent organization to verify the quality of a product or its ingredients, but that does certify the safety or effectiveness of a product. Due to high dilution of ingredients in homeopathic medicines they do not create much problem for baby.
